We have this document on hand when members are on site for various meetings and gatherings (including committee meetings, small conferences, etc.) where we'll be doing photo capture (including capturing photos for marketing and promotions or headshots). We don't use this form for large gatherings, such as our annual meeting. At large events, we capture a photo release via a required check box in the registration process.
